approximately how wide is the stone in inches?

Hi!:) I think it varies, as I have ordered 2 stones from these guys and one is about 1&1/2 inches wide (in the middle, where it's widest) and the other is larger - maybe a tad over 2 inches. :)

What do If receive everything but not Shiva?

I have purchased many from this vendor. All are authentic Shiva Lingham. Naturally smooth river rocks from one river in India. Yes you have to trust them to pick one out for you, and they are all different. Not sure what you are referring to as the shiva characteristic, but they represent a balanced male and female union.
